I don't understand how the terrible relic of sickness works

I bought it during the Halloween event, following the recommendation of some guides for my class. But I don't understand how it works.

The conditions for it to work are normal; the player or mob has to stay still. But even when I meet this condition, something happens that I don't understand. It seems like the enemy's health bar has to be full—not the health itself, just the bar—for it to start working. But in the process, I lose a few seconds where it could be dealing damage. Moreover, in these circumstances, especially if it's a boss, I would be dealing more damage than this relic fills the health bar, which makes it end up not dealing any damage at all.

Below are some test videos. When the damage dealt is high, the relic is practically useless. When it's low, it works to some extent. However, as seen in both videos, I didn't attack when it activated, which normally doesn't happen in real combat.

What am I reading right now? You guys are confusing it with Life Weakness relic.

This relic does not deal damage, it periodically reduces enemy's max HP by 2% , hence why you see the dummy's HP bar going up (Max HP is reduced). You can see at the end, the dummy's HP became 30k (lost 40% total).

 

It can be good at the beginning of a boss fight, where you can reduce, let's say, the boss's HP from 100k to 60k in 20 seconds. Which could be faster than your damage, but if your damage is more than 2% of the boss's HP per second, then you won't really see its effect as you pointed out, so it's not really always useful in PvE.

In PvP, it is useful in prolonged fights, where the enemies either rely on HP for the power of some skills (e.g. Paladin) or against everyone really that keeps healing up to max HP time and time again, so you can get a better chance in killing them the next time.

Hello! The relic inflicts some damage to the enemy while they are standing still [often while they are attacking you]  

 

Yeah the description isn't very clear but that's what it does.

The damage it does is based on the targets max hp. It won't do much against weak enemies, but in pvp if you land it on a tank class, or in PvE if you land it on a high hp boss, it's very effective.

It does virtually do damage since the target won't gain back the hp lost even if their max hp goes back up. For example a 400k hp boss will go down all the way to 240k, which wont be recovered when the relic ends, which is far more damage than a lot of non damage focused classes can do. Hence why I said it works very well against high hp bosses, especially if you're soloing.

 

Similarly it works as a great way to do some extra damage at the start of the fights against tank classes, which can be difficult to wither down.
