Пойду продам слиток, куплю кружечку, налью туда пару зелей, вылью чернила, напьюсь до отвала и лягу на свитки, а обезьянка меня утром разбудит, ну или вызовет скорую если уж совсем чудно будет. Ну а арбалет, для возможной охоты на обезьянку, если белочка разбудит меня быстрее обезьянки, а жабка будет снимать этот концерт на камеру!

Putting aside the fact that it's kind of a funny situation, the most plausible thing happened is that this unfortunate fellow has joined the arena fight right when the popup of his Empty O2 tank appeared.    A "No decision taken" input implies having decided not to refill it (this input is taken into account even when the closure of the popup gets "forced" - like exiting the game or, in fact, entering a new area like the Arena) and thus death by drowning.    Or more simply, it might be a bug connected to this scenario.
