It means that you died like a boss in the middle of enemy free location.You see, not every game situation can be explained, maybe it's some kind of glitch or something, we have no mass reports about such situations so looks like it's just an unlucky occasion.

First one, it's ok, if you have problems like the one mentioned you should write to support.
Second one, you got your answer, "if you don't know the rules it doesn't mean that you are not responsible for violating them", that's the answer.
Third one is not even closed, what's a problem?
