I have experienced this bug really irritating and annoying so much and make the pleasure of the game gone. I hate it when I need to kill lots of enemies to get peace while collecting and then there's nothing. Let's say one quest item has drop rate set to 80%, the other one 70% - if you hit both at once, remaining 20% of first and 30% of second, there is no item. Simple as that. It looks like a mushroom but oh no its only a picture :pardon: . Its better if the image is crystal so its really crystal not an fth empty spot. They are nothing special like some "rare items" and they should always be there. Those empty fields are really unnecessary. They are really just annoying.
